- For websites: This must include the website address.
- For apps: This must include everything on the screen.

Additionally, the screenshot/photo must display:

- For email: This must include: to, from, subject and date fields.
- For text messages: This must include the name/number of the sender.

All submitted test PDFs/images must contain your full name and the date of your positive COVID-19 test.

When you will receive your certificate

Your certificate will be emailed to you as a PDF within your chosen timeframe if your application is successful. Certificates are issued during working hours (Monday to Sunday 9am-5pm)

Covid-19 Recovery Certificate Type

Express Delivery £70.00- You'll receive your certificate within the next 20 Minutes (within our normal business hours)

Standard Delivery £55.00 - You'll receive your certificate within the next 2 hours (within our normal business hours)

I am the patient (or) their legal guardian/representative/parent, that the details entered are true and correct to the best of my knowledge, and the test certificate/notification is an original and has not been edited in any way

I confirm that I have reviewed the guidance of the destination, country or organisation and that my symptoms and test meets their requirements

I understand that I must follow the covid guidelines (including the dates for isolation) of my origin (departure) country

I understand that if my test, or the start of my symptoms, was less than 11 days ago, that I must wear a mask at all times when I am travelling
